Understand that martial arts by its very nature involves body contact.  
It is physically and mentally demanding.  The equipment and
weapons that are associated with the study of martial arts
represents a certain risk to the user.  It is recommended that you
check with your physician prior to participating in martial arts
activities.
I understand that I am to have liability, and health insurance for the
participant I have listed above.  I am aware that injuries occur and
that the participant may need to be taken to the emergency room for
treatment on some injuries.
In consideration of my acceptance into White Tiger, I hereby
